MK-677 is not a peptide. It is a small molecule taken by mouth that acts like ghrelin, the hunger hormone, and makes the body release more growth hormone.4, 15 It has been tested in randomized trials in older adults, in hip fracture recovery and in Alzheimer's disease. It raised growth hormone and lean mass, but strength and function didn't improve in older adults. A hip fracture trial found faster walking but no gain in most other measures, and was stopped early for heart failure, Blood sugar rose in several trials.3, 4, 5 It is not approved, can't be compounded and is not a legal supplement.1, 10

Often discussed alongside growth hormone peptides such as ipamorelin. See the peptide regulatory tracker for how they compare legally.

Where to get it

MoveWell sells nothing and earns nothing from this page. There is no legal way to buy MK-677 in the US. Why

Why

Molecular structure

A small molecule, not a chain of amino acids. A spiropiperidine ring system carries a methanesulfonyl group and is joined by amide bonds to two modified amino-acid units.

MK-677 molecular structure: a spiro-indoline-piperidine ring system with a methanesulfonyl group, linked by amide bonds to an O-benzyl serine unit and an aminoisobutyric acid unit, with the ring system highlighted

What it is

MK-677 was developed by Merck in the 1990s as a pill that raises growth hormone. Researchers describe it as an "oral ghrelin mimetic": it switches on the same receptor as ghrelin, the hormone that drives hunger and growth hormone release.4

It is often sold as a "peptide" or grouped with SARMs, but it is neither. Lumos Pharma, which now develops it as LUM-201, calls it "an orally administered small molecule."15 That's why it works as a pill, when most peptides have to be injected.

How it works

  • Ghrelin receptor. It binds the ghrelin receptor in the brain and pituitary, which triggers pulses of growth hormone.4
  • IGF-1. More growth hormone raises IGF-1 from the liver. In trials IGF-1 rose by about 40% to 80%.7, 8
  • Appetite. Because it mimics the hunger hormone, appetite rises, at least for the first few months.4
  • Blood sugar. Growth hormone works against insulin. Fasting glucose rose and insulin sensitivity fell in trials.4, 8

Unlike most compounds on this site, these effects were measured in people in randomized trials.

Evidence map

Each claimed use is graded separately. A grade measures how much evidence exists, not whether the result was good news.

Raising growth hormone and IGF-1

Consistent in several small and mid-size randomized trials. A lab number, not a health outcome

Lean mass in older adults

One trial of 65 people: +1.1 kg lean mass at 12 months, but no gain in strength or function. Too small to test function well

Recovery after hip fracture

One trial of 123 people: faster walking and fewer falls, most other measures unchanged. Stopped early for heart failure

Alzheimer's disease

One 12-month randomized trial. No effect on the disease

Growth hormone deficiency

Small short studies raised IGF-1. A phase 3 trial in children is under way

Muscle or strength in young, healthy people

No study found. User reports only

Scale: 5 FDA-approved, 4 strong human, 3 limited human, 2 animal or lab only, 1 anecdotal

How it's used

No dose is approved for any use. In trials it was a pill taken once a day, in older adults or patients under medical supervision.3, 4 Those trials tracked blood sugar, heart failure and other side effects; buying it online skips all of that. We don't give a dose.

Side effects and who should avoid it

  • Heart failure. The hip fracture trial "was terminated early due to a safety signal of congestive heart failure."3 FDA says ibutamoren "poses significant safety risks due to the potential for congestive heart failure in certain patients."1
  • Blood sugar and insulin resistance. Over 1 to 2 years in older adults, fasting glucose rose and insulin sensitivity fell.4 Glucose tolerance worsened in obese men within 2 weeks.8 People with diabetes or prediabetes have the most to lose.
  • Water retention. Mild, short-lived swelling of the legs was among the most common side effects in the older-adult trial, with more appetite and muscle pain. FDA also lists water retention.4, 9
  • Appetite. Appetite rose and then eased within a few months. Weight went up 2.7 kg, versus 0.8 kg on placebo.4
  • Other. Muscle pain, fatigue and a rise in cortisol.4, 9 FDA says "long-term effects of ibutamoren use are unknown."9
  • Hidden in products. In 2025 FDA found unlisted ibutamoren in a growth formula sold for children aged five and older.9
  • With SARMs. FDA warns ibutamoren may interact with SARMs in "potentially harmful" ways.9
  • Athletes and service members. Banned by WADA at all times, and on the Defense Department's prohibited supplement ingredient list.11, 12
  • Pregnancy, breastfeeding and children. No safety data outside the pediatric trial.

FAQ

Is MK-677 a peptide?

No. It's a small molecule taken by mouth that acts on the ghrelin receptor. It's sold alongside peptides, which is why it's in our library.

Is MK-677 legal?

It isn't FDA-approved, pharmacies can't compound it, and FDA says it isn't a lawful supplement ingredient. The only legal use in people is in a registered clinical trial.

Did the 2026 HHS peptide policy change anything?

No. The policy covers six peptides: BPC-157, TB-500, epitalon, semax, KPV and MOTS-c. MK-677 isn't one of them.

Does it build muscle?

In older adults it added about 1 kg of lean mass over a year, but strength and function didn't improve. No trial tested it in young, healthy people.

Is it safe?

It's been tested in people, but the trials raised safety concerns. A hip fracture trial was stopped early for heart failure, and blood sugar rose and insulin sensitivity fell in several trials.

What dose do people use?

No dose is approved for any use. We don't give one.

Can athletes use it?

No. WADA lists it under S2.2.4, banned in and out of competition, and Sport Integrity Australia says no therapeutic exemption would be granted.

Is it being developed as a drug?

Yes. Lumos Pharma is testing it as LUM-201 in a phase 3 trial in children with growth hormone deficiency.
