Best TENS Units in 2026, by Evidence

TENS unit picks by use against fixed criteria, with the FDA record for each pick, graded evidence for period pain, fibromyalgia, back pain, knee arthritis and labor, and who should not use one.

We may earn a commission from links on this page. It never decides our picks. How we make money

Last verified November 2, 2026. Every FDA number, grade and price on this page links to the record or page we checked. Where we couldn't confirm something, we say so.

We have not tested these units. Specs are the makers' own; we found no independent output measurements for any of them.

The short answer

A TENS unit can take the edge off pain while it is running and shortly after. That is the most consistent finding across hundreds of trials1. The evidence is best for period pain and movement-related pain in fibromyalgia, mixed for labor, and weak or negative for chronic low back pain and knee osteoarthritis. Most over-the-counter units do the same basic job. Buy a cheap, FDA-cleared unit first, and only pay more for a feature you will use, like two channels, a wearable design or EMS programs. Don't use one if you have a pacemaker or epilepsy2.

Status snapshot: what FDA clearance covers

Over-the-counter TENS units are Class II devices under FDA's TENS regulation3. Most reach the market through a 510(k), which shows the device is "substantially equivalent" to one already sold. A 510(k) does not prove the device works for any particular condition. These are the records we found for the units on this page.

Record typeWhat it coversExamples we looked up
510(k), product code NUH (over-the-counter TENS)Temporary relief of sore, aching muscles; some labels add chronic painOmron PM400 (K181992)4, Omron PM500 (K182120)5, Livia (K163153)6, Tenscare Ova+ (K230926)7
510(k), product code NYN (TENS for arthritis)Arthritis pain, usually as a second code on the same clearanceOmron PM4004 and PM5008, iReliev ET-70709
510(k), product code NGX (muscle stimulator)The EMS side of combination units: stimulating healthy musclesiReliev ET-70709
De Novo, prescription onlyFibromyalgia symptoms in adults with high pain sensitivityQuell Fibromyalgia (DEN210046)10

Read the indication, not the marketing. Livia sells itself as a period-pain device, but its 510(k) lists general uses: chronic intractable pain, sore muscles from exercise or housework, and muscle stimulation6. That is normal for this category. The clearance tells you the device is a standard TENS unit. The trials below tell you whether TENS helps your kind of pain.

Evidence map: graded by condition

Grades: A consistent randomized trials or meta-analyses. B supported by randomized trials that are small, varied or short. C few, mixed or mostly negative trials. D lab, animal or anecdotal only. X a safety reason not to use it.

ConditionGradeBest evidenceWhat it means
Pain in general, during and right after useBMeta-analysis of 381 randomized trials, 24,532 people: pain was lower than with placebo during or immediately after TENS (SMD −0.96), moderate-certainty evidence. Side effects were mild and poorly reported1.TENS works while you use it. Most trials didn't test lasting relief.
Period pain (primary dysmenorrhea)B−Cochrane review, 20 trials, 585 women: high-frequency TENS (10 trials, 345 women) and low-frequency TENS (3 trials) may reduce pain vs placebo or no treatment. Low-certainty evidence11.The best-supported specific use. Trials are small and at risk of bias.
FibromyalgiaB−Placebo-controlled trial, 301 women: movement-evoked pain fell 1.0 point more than placebo TENS on a 0 to 10 scale after 4 weeks12. A 2026 trial across 28 physical therapy clinics (384 people) found 1.2 points less pain with movement after 60 days when TENS was added to physical therapy, but had no placebo group13. A 2017 Cochrane review had found the evidence too weak to judge14.Newer trials are positive for pain during activity. One prescription device is authorized for this use10.
Labor painCCochrane review, 17 trials, 1,466 women: pain ratings were similar with and without TENS. TENS at acupuncture points lowered the chance of severe pain in two studies. Most women said they would use it again, and no adverse events were reported15.Low risk, uncertain benefit. Use only with your midwife or doctor's agreement.
Chronic pain over weeks or monthsCOverview of 9 Cochrane reviews (51 trials, 2,895 people): the authors could not say with confidence whether TENS relieves chronic pain16.Short-term relief is clearer than long-term benefit.
Chronic low back painCCochrane review, 4 placebo-controlled trials, 585 people: the evidence doesn't support routine use17. NICE advises clinicians not to offer TENS for low back pain or sciatica18.Some people still find it helps. Don't expect it to replace exercise therapy.
Knee osteoarthritisCDouble-blind trial, 220 people, 3 weeks: no difference from placebo TENS in knee pain (WOMAC pain difference −0.06) or function. The authors called further similar trials unnecessary19.The largest placebo-controlled trial is negative.
People with a pacemaker or epilepsyXThe NHS says not to use TENS if you have a pacemaker or epilepsy, or if you are pregnant unless your doctor or midwife tells you to2.Ask your doctor first.

The pattern: TENS is a short-term pain tool. It helps most when you use it during the activity that hurts, as in the fibromyalgia trials, or during the hours of period pain. It doesn't treat the cause.

Picks by use

We pick by use, then name a unit that meets all four criteria. The criteria apply to every brand, whether or not it pays us:

  1. The unit, by model number, appears in an FDA record we could look up (a 510(k) or De Novo number).
  2. It is sold over the counter, unless the use only has a prescription device.
  3. We checked the price on the date shown, and the warranty or return terms are published.
  4. Commission plays no part.

Expensive units don't have better evidence behind them. The trials above used ordinary TENS settings, so a basic cleared unit is a fair test of whether TENS helps you.

Budget, one area: Omron Pocket Pain Pro (PM400)

Cleared under K1819924. Single channel, preset programs for arm and shoulder, lower back and leg and foot, plus massage-style modes, with 10 intensity levels. Omron's page is inconsistent about the exact number of pain modes. MSRP $51.99 on November 2, 2026, with a 1-year warranty on the unit and 30 days on the pad cord20. Good for trying TENS on one sore area before spending more.

Simple and stronger: Omron Max Power Relief (PM500)

FDA's device record lists the PM500 under K1821205. That clearance names the earlier model PM3032B and includes an arthritis mode8; Omron says the PM500 replaced the PM3032. It has six body modes (shoulder, lower back, arm, foot, leg, joint) and three massage modes, with 15 intensity levels. Omron says it has twice the power of the Pocket Pain Pro and sets a 15-minute maximum per session. MSRP $79.99 on November 2, 2026, with a 1-year warranty21. Still one channel.

Two channels, TENS plus EMS: iReliev ET-7070

Cleared under K141299 for over-the-counter TENS, arthritis pain and muscle stimulation (codes NUH, NYN and NGX)9. Two channels let you treat two areas, or a larger area, at once. It has 14 programs (8 TENS, 6 EMS) and 25 intensity levels, and runs on 3 AAA batteries rather than a rechargeable battery2223. It was $79.95 (regular $99.95) on living.fit on November 2, 202622. Wareable reports a 1-year warranty23. See our TENS vs EMS guide if you're unsure you need the EMS side.

Period pain, wearable: Livia

Cleared under K163153 as an over-the-counter TENS device6. As noted above, the clearance doesn't name period pain, but TENS for period pain has the best condition-specific evidence on this page (grade B−)11. It was $129 on Walmart (sold by iPulse Medical) on November 2, 202624; mylivia.com showed no price when we checked. Livia's FAQ gives a 2-year warranty and a 120-day refund window on one-time purchases, but subscription payments are non-refundable outside the EU. It says not to use Livia with a pacemaker or during fertility treatment25.

An alternative is the Tenscare Ova+ (K230926)7. It was £37.50 on Tenscare's UK store on November 2, 2026, and US orders may pay duties and a handling fee26.

Fibromyalgia: Quell Fibromyalgia (prescription)

Quell Fibromyalgia is the only device we found with FDA authorization for fibromyalgia. It got De Novo authorization in May 2022 as "an aid for reducing the symptoms of fibromyalgia in adults with high pain sensitivity". It is prescription-only, can be worn during sleep, and works only with NeuroMetrix electrodes10. electroCore bought it from NeuroMetrix in May 2025. In May 2026 electroCore listed it among its prescription products and said an over-the-counter Quell Relief, for lower-leg pain, would follow in the second half of 202627. We couldn't open the Quell store to check prices, so there is no buy link. Ask your doctor. The trials behind its fibromyalgia use are in the evidence map1213.

Not picked

  • Wireless units. None met criterion 1. The iReliev Wireless (ET-5050) is described as FDA-cleared, but we couldn't match the model to a K number28. The Compex Wireless USA 2.0 lists four K numbers, but it is mainly a muscle stimulator; see our TENS vs EMS guide.
  • TENS 7000 2nd Edition. A popular cheap dual-channel unit ($38.88 at US Med Rehab on November 2, 2026)29. FDA's record links K110707 to the TENS 7000 model DT607030, but we couldn't match the current DT7202 model to a clearance.
  • Auvon. Auvon sells many similar units from different factories. We couldn't confirm which current model matches which clearance.
  • Therabody PowerDot 2.0. Best Buy lists the PowerDot Duo as no longer available new31.

Specs and prices side by side

Price checked November 2, 2026. These are the makers' specs. We found no independent lab measurement of output for any of these units, so there is no measured column. Pads are an ongoing cost for all of them.

How to choose a TENS unit

Start cheap

Whether TENS helps you is the main question, and a basic cleared unit answers it. The 381-trial meta-analysis found pain relief across many conditions, mostly with standard settings1. Upgrade only if you need a feature.

One channel or two

One channel runs one pair of pads. Two channels let you cover both sides of the back, or two separate areas. If your pain is in one spot, one channel is enough.

High or low frequency

Most units offer both. In the Cochrane review of period pain, both high- and low-frequency TENS reduced pain compared with no treatment, and it's unclear which works better11. Try the presets, then keep the one that helps.

TENS only, or TENS plus EMS

EMS programs contract muscles. They don't add pain relief. Buy a combination unit only if you also want muscle stimulation for training or rehab. See TENS vs EMS.

Check the FDA record yourself

Ask the seller for the K number and search it in FDA's 510(k) database. Check that the model you're buying is named. "FDA-registered" means only that a company is listed with FDA. "FDA-approved" is almost never accurate for TENS units.

Safety: who should avoid TENS and where not to put pads

These are the NHS's rules2. Your device manual may add more.

  • Don't use TENS if you have a pacemaker or epilepsy, or if you're pregnant, unless your doctor or midwife says you can. Labor TENS is used on the back, and only with their agreement215.
  • Don't put pads on your neck, mouth or eyes, on your chest and back at the same time, or on irritated, broken, infected or numb skin.
  • Don't use it while driving or operating machinery, in the bath or shower, or while asleep. Quell Fibromyalgia is the exception: it is labeled for use during sleep10.
  • Skin reactions are the common side effect. If your skin gets itchy, red or irritated, stop and remove the pads.
  • Check with a doctor first if you aren't sure TENS is safe for you, or if pain is new, severe or unexplained.

Across 381 trials, side effects were mild and no serious ones were attributed to TENS, but they were poorly reported1.

Myths

  • "FDA-cleared for pain means proven for my condition." Most clearances name general muscle pain. Whether TENS helps a specific condition comes from trials, and for knee osteoarthritis the largest placebo-controlled trial was negative19.
  • "TENS heals the injury." Trials show relief during and right after use1. There is no good evidence that it repairs tissue.
  • "TENS and EMS are the same." TENS targets nerves for pain relief. EMS makes muscles contract. Many units do both.
  • "A pricier unit works better." Trials used ordinary TENS settings. We found no evidence that premium units relieve more pain.

FAQ

Do TENS units actually work?

For many people, yes, for as long as the unit is on and shortly after. A meta-analysis of 381 trials found less pain than placebo during or right after use1. Long-term benefit for chronic pain is uncertain16.

Does TENS help period cramps?

Probably. A 2024 Cochrane review found that both high- and low-frequency TENS may reduce period pain compared with placebo or no treatment. The evidence is low-certainty11.

Is TENS good for lower back pain?

The evidence is weak. A Cochrane review didn't support routine use for chronic low back pain17, and NICE advises clinicians not to offer it18. Some people still find it helps during flare-ups.

Can I use a TENS unit while pregnant?

Only if your doctor or midwife says so2. TENS is sometimes used in labor. In trials, it was safe but made little difference to pain scores15.

Do I need a prescription?

Not for over-the-counter units like Omron, iReliev or Livia. Quell Fibromyalgia, the only device authorized specifically for fibromyalgia, needs a prescription10.

How long can I use it per session?

Follow your device's manual. Omron, for example, sets a 15-minute maximum for the Max Power Relief21.

Does TENS help knee arthritis?

Probably not much. A double-blind trial of 220 people found no difference from placebo TENS after 3 weeks19.

FDA records were checked on November 2, 2026 through FDA's GUDID database (directly or via beudamed.com) and 510(k) data mirrored by fda.innolitics.com and regdatalab.com, because FDA's own 510(k) search page didn't return the right records.
